Woven Health Clinic, formerly Metrocrest Community Clinic, is a 501c3 charitable clinic that provides primary health care services and is a medical home for people who live in Addison, Carrollton, Coppell, and Farmers Branch, Dallas and surrounding areas that cannot afford healthcare.

 

Health Services of North Texas, Inc. (HSNT) is a nonprofit organization dedicated to providing excellent medical care for the entire family from newborns to grandparents including prenatal care, counseling services, as well as chronic and infectious disease care management.

At Health Services of North Texas, our staff is committed to providing compassionate service using an approach that provides the highest level of care, while giving our patients the tools they need to take charge of their health. HSNT is a responsive organization with a long history of providing for the changing needs of our communities.

Do you need free medical care?

We do not provide emergency or trauma care at our Clinic, but can provide basic medical care and health education for you and your family. We are a free and charitable medical clinic only to those without medical insurance or financial means to pay for basic healthcare services. Donations are always, graciously accepted to replenish for our supplies.

When you visit the clinic, you’ll also have an opportunity to participate in the community programs, classes, and events offered at our location on Wednesday nights. Learn more about these by visiting the 2435 Kinwest Community Night webpage.

Anyone is Welcome

  • Services are free only those without medical insurance and those without financial resources to pay for their basic healthcare services.
  • A current, valid ID is required for ALL patients.
  • A donation to replenish our medical supplies is always welcomed, needed and so appreciated!
  • Spanish is spoken
  • No residency requirements
  • No appointment necessary, walk in only. First come, first served.
  • Check-in is from 2:30 – 3:30 p.m. and is required for anyone needing to be seen.

  • High blood pressure
  • School sports/participation physicals will be given at Clinic. Student must provide their SCHOOL FORM and if under age 18 a parent/guardian must be present during registration/triage/exam. A donation of $5.00 to $10.00 is suggested for school physicals in order to replenish Clinic diagnostic and administrative supplies.

Baylor Scott & White Community Care – Carrollton is devoted to the total health care of uninsured and under served people of Carrollton and its surrounding communities.  We provide continuing medical care for individuals and their family members of all ages, from children 5 and older to the elderly.

Our expertise is based on the knowledge of our patients in the context of their community.  This community has a very high incidence of uninsured residents, many of which suffer from chronic diseases such as diabetes.  Our primary focus is on providing a medical home to those residents that are in need in order to assist in their overall health and wellness.Appointment Information Click here for more details regarding making an appointment.

Baylor Scott & White Community Care – Carrollton offers comprehensive care for patients of all ages. Our physicians and staff are able to provide a variety of health services allowing patients to receive all of their health care needs within our office.

We welcome new patients and can often accommodate same day appointments. Making an Appointment On your first visit, we will begin a permanent record. Please bring any medications you are taking, so that we may accurately determine the dosage and frequency. We will ask you to complete a medical history and registration form. Please bring your insurance card and try to arrive at least thirty minutes early for your first visit. If you have been under the care of another doctor, we may ask your permission to transfer some of your records.

We have three appointment categories that take up varying amounts of time:

  • Annual Exams or New Patient Visits – Plan on spending one hour including check-in time, filling out forms, and the actual physician visit.
  • Follow-up Exams – Thirty minutes
  • Sick Visits – Varies depending on the illness, plan on at least thirty minutes.
